Applications for Perry Township staff will open January 9, 2023. Non-staff applications will open January 16, 2023.

23-24 Tuition Rates:
Staff Members: $175 per week
Non-staff members: $200 per week

You can view/print your registration packet below. Please print out the forms and fill them in. You can mail your pack it or drop it off at the preschool (6548 Orinoco Ave. Indianapolis, IN 46227). You will need to complete all of the following forms to register your child for preschool:

1. Print, fill out, and return a fully complete 2023-2024 Updated Enrollment Application Packet
2. Print, fill out, and return 2023-2024 Emergency Consent Form
3. Print, fill out, and return 2023-2024 Home Language Survey
4. Print, fill out, and return 2023-2024 Race Ethnicity Survey
5. Print, fill out, and return 2023-2024 Parent Contract (English and Chin)
6. Print, fill out, and return 2023-2024 Discipline Policy (English and Chin)
7. $75 registration fee (due at time of registration, exact cash/check only)
8. $35 cooking fee (due at time of registration, exact cash/check only)
9. Birth Certificate
10. Shot records
11. Proof of residency (ex. utility bill)
12. Proof of income (for Title 1 support only)
