Please check your child’s shot record. If your child is entering kindergarten, 6th grade or 12th grade, a booster shot is required.

If your child still requires immunizations, make an appointment at your doctor’s office or with the Marion County Public Health Department. The health department clinics provide free vaccinations for people who have Medicaid Part A, and charge a flat fee of $10 for all others. There are walk-in hours and appointment-only hours at two local clinics:

On Monday, August 17th, 2015, Perry Township Schools will exclude students who are not current with vaccinations from attending school. This is in accordance with Indiana state law.
