- Proof of Residency (2 of the following) – current (dated within 30 days) utility bill, bank statement, pay stub, government benefits letter, or insurance statement with name of guardian and address; letter from employer on letterhead documenting address update; or online receipt from US Postal Service documenting address change. Not acceptable: a lease, a driver’s license, and other random mail.
- Birth Certificate
- Guardianship paperwork if different than the birth certificate
Necessary, but not Required:
- Immunization Records
- Last report card
- Previous test scores for standardized tests and local progress monitoring tests
