PMMS Athletics has policies in place to keep our coaches and athletes safe. Please be aware that these policies and procedures are mandatory for our athletes and coaches to follow. We are following the Indiana DOE and the IHSAA recommendations, while also staying consistent with our high school. Please note that these procedures and start times remain tentative based on current health information provided by the state of Indiana, the ISDH and the IHSAA.

ALL parents of athletes in grades 6 – 8 need to register their students in Final Forms prior to participating in an athletic season. Any athlete who attends practice without being fully eligible in final forms will not be eligible to participate.

HOW TO CREATE AN FINAL FORMS ACCOUNT

Once the athlete is registered they will need to do the following:

1. Sign into their school email account.
2. Click on the email, from Final Forms, to verify their email account or sign into their already existing Final Forms account.
3. Sign the required student forms.
4. Finally once the Physical Form is completed and signed by the doctor, you will upload it into Final Forms by scanning or uploading a photo of each page.

All athletes must have a completed IHSAA physical on file dated after April 1, 2024.

IHSAA PHYSICAL

• If an athlete is not feeling well, please do not send the athlete to a workout.
• All students should bring his or her own LABELED water to workouts. There may not be access to outdoor water filling stations. Indoor water bottle filling stations will be accessible.

Please contact the coaches listed for any questions you may have about an individual sport. It is very important that you complete the final forms registration and IHSAA physical requirements prior to the first day of participation.
