Medication Consent Form & Policy
Your written permission is required, in advance, if your child has a prescription or over-the-counter medicine, or requires management of chronic health conditions or any health needs with a physician order while in school. Click on the links below to obtain the medication consent form in English, Chin, and Spanish, or to read our Board policy related to medications in its entirety. Before bringing any medications to school for your child, check the expiration date. All medications must be in their original containers and prescriptions must have the pharmacy label affixed. Parents must bring medications to school for students in grades pre-K through 8. Students in grades 9 through 12 may bring medications to school.
