Secondary Transfer

Please note: If a transfer form is approved, a new form does not need to be completed each year unless your demographics change. This applies to grades 6-12.

INTRA-DISTRICT (RESIDENCE IS WITHIN PERRY TOWNSHIP BOUNDARIES)

INTRA-District transfer applications will be accepted year-round for students currently attending school within Perry Township.

Complete your Intra-District application by following the steps below:

  1. Download Form Here
  2.  Save and complete the application on your computer
  3.  Submit the application (Choose an option below):
  • Email completed applications to cford@perryschools.org
  • Print the application and deliver it to the building you are requesting to transfer to

NON-Resident Out of District (RESIDENCE IS OUTSIDE PERRY TOWNSHIP BOUNDARIES)

For middle school and high school students currently attending school and/or with residence outside Perry Township, transfer applications will be accepted until August 28, 2026 for Semester 1 and until January 11, 2027 for Semester 2. Applications received after these dates will be considered for the following semester.

Complete your Inter-District application by following the steps below:

  1. Download the form
  2. Save and complete the application on your computer
  3. Assemble the following documents for your student’s application
    Grades 6-8: Recent report card, attendance report, and discipline report
    Grades 9-12: Up-to-date transcript, attendance report, and discipline report
  4. Submit the application (Choose an option below)
  • Email completed applications to cford@perryschools.org
  • Print the application and deliver it to the building you are requesting to transfer to
